The Ho-Chunk Health Care Center (HHCC) was opened in 1989 and was further expanded to its present facilities in 2002/2003. This facilities first obtained accreditation from the Accreditation Association for Ambulatory Health Care (AAAHC) in 2003. In pursuit to maintain accreditation with AAAHC, the HHCC underwent a more intense evaluation of its clinic for the recertification process and this clinic had to show it adhere to more stringent standards of care. In 2006, the HHCC was awarded recertification of accreditation for another three-years. In 2009, the HHCC received its re-certification of accreditation for another three-years.
Clinic Hours:
The Ho-Chunk Health Care Center is open Monday - Friday, 8:00 a.m. to 4:30 p.m., except tribal holidays. (See Pharmacy for adjusted hours.)
Clinic Staff
We have doctors, nurse practitioners, registered nurses, laboratory and radiology technologists, pharmacist, pharmacy technician, optometrist, podiatrist, exercise physiologist and registered dietician. We also have many support staff such as those working in medical records, transcription, billing, registration, and scheduling for full clinic services.
Doctors
Our doctors can provide a wide range of services and health care. These include general internal medical, family practice, and pediatrics.
Nurse Practitioners
Our Nurse practitioners' specialties are family practice and primary care which include women's health, pediatrics, geriatric. These nurse practitioners can provide a broad range of health care services, which include:
- Taking the patient's history, performing a physical exam, and ordering appropriate laboratory tests and procedures.
- Diagnosing, treating, and managing acute and chronic diseases.
- Providing prescriptions and coordinating referrals.
- Promoting healthy activities in collaboration with the patient.
Nurse Educator
Nurse educator provides a variety of patient education materials on topics such as: diabetes, pre-natal education asthma, smoking cessation, cholestral, and high blood pressure. The nurse education program is to provide information and support to patients with health concerns, enabling them to manage a condition and live a life of optimal health. Patients can request information or make individual appointments to meet with the nurse educator.
Registered Nurses
Our registered nurses assist the medical team in providing care and health education to all clinic patients. These nurses are available during regular clinic hourse to answer your question or to refer such matters to a health care provider, when needed.
Optometry
The Optometry Clinic is staffed by an optometrist and an optometric technician. Optometry clinic offers comprehensive eye examinations and contact lens services. Adjustments and minor eyewear repairs are also available. A wide variety of frames and lenses are available through the dispensary at reasonable cost.
Pharmacy
We have a full-time pharmacist and certified technician operating our clinic pharmacy. The pharmacy hours are adjusted for patient pick-up convenience, Monday through Friday, 8:30 a.m. - 5:00 p.m.
Podiatry
The podiatrist on staff is able to perform most services from routine care to surgery. He has limited hours, available on every other working Monday.
X-Ray services
X-Ray services are provided at HHCC. Patients must see a provider who will decide whether an x-ray is needed. X-Ray studies include: chest, abdominal, upper and lower extremities, and the spine. Patients requiring CT's, MRI's or unltrasounds will be referred to another facility since HHCC cannot perform these services. Mammography clinics are held monthly by appointment.
Laboratory services
Laboratory services are provided at HHCC. Many routine lab test are done at the clinic. Those tests that cannot be performed here are sent to an outside reference laboratory.

Please arrive 15 minutes (30 minutes for new patients) prior to your scheduled appointment. This allows time to fill out the necessary paperwork. Remember to bring in your insurance card, Tribal ID card, and social security card.
