NOTICE:
THE ELECTRONIC ADDRESS VERIFICATION WILL BE CLOSED FOR SUBMISSION OF THE 2019 ANNUAL ADDRESS VERIFICATION FORM AT 4:30 PM ON DECEMBER 16, 2019.
After this time and date, you will need to submit a paper AVF. Please contact the Enrollment office at enrollmentinfo@ho-chunk.com or by calling the Office for a reprint.
REMEMBER: If you submitted your 2019 AVF using the Electronic submission you are not going to receive a paper AVF in 2020. Information and instructions for filing your annual AVF will be mailed on or before June 1, 2020.
Notice will appear on this website when the 2020 Electronic AVF is ready for submission.
