In-Person Tribal ID
Come in-person to the Ho-Chunk Nation Tribal Office Building located at W9814 Airport Rd, Black River Falls WI, 54615. Our office is open Monday-Friday from 8 AM to 4:30 PM and available to do walk-in or appointments to get a tribal ID printed the same day.
What you need (for adults): An ID Document
What you need (for dependents):
- A legal guardian present
- A legal guardian ID document
- An ID document for your dependent
What qualifies as an ID document?
- A certified birth certificate
- Drivers license
- State ID
- Passport
- Tribal ID (current or expired)
Mail-Order Tribal IDs
Ho-Chunk Nation Tribal ID’s are currently free to process and ship.
To get a Mail Order Tribal ID we need: Information Request Form, an ID, Photo, and Signature.
Accepted forms of ID can include
- Legible photograph of certified birth certificate
- Drivers license
- State ID
- Passport
- Tribal ID (current or expired)
For your Picture
For your photo we can only accept the following:
- Photo of yourself (whole head should be visible along with some shoulder)
- Plenty of light
- White or light blue background.
- No hats or sunglasses
- No heavy image filters or AI editing
- Sent as an image file like JPG not a PDF
Update your Signature
- Dark Signature on a white piece of paper.
