The Ho-Chunk Nation and the Enrollment Division are working to provide services to tribal members while maintaining a COVID-19 safe environment to stop the spread of the pandemic. The appointment times are allotted to 25 minutes per person to allow for proper sanitizing.

Request for Tribal ID ~ Change of Address form ~ driver’s license ~state ID ~ passport ~ expired tribal ID
Name Change documents refers to:
Request for Information form ~ Certified Marriage Certificate ~ Copy of Divorce Document ~ Copy of Social Security Card with new name
Tribal ID Fees
- Duplicate Tribal ID Cards are $20.00
- Expired Tribal ID Cards are $20.00
- Elder Tribal ID Cards and First Tribal ID Cards are free
- Accepted forms of payment: Money Order & Cashier’s Check ONLY (No Personal Checks)
- Cash may be accepted at the time of the appointment if you arrive 15-minutes early for processing
- All payments must be received prior to issuing the Tribal ID
Temperature reading is required upon arrival. A face mask is required before and after taking your picture. Tribal members must be symptom free.
If you are experiencing COVID-19 symptoms or were exposed, please cancel and reschedule your appointment as soon as possible. Cancellation can be done by following the link on your confirmation email or calling 715-284-9343 ext:11015.
